The following documents are required for registration:

Child's Birth Certificate

Parent/Guardian photo ID

Proof of Residency (current utility bill, lease, mortgage statement, bank statement, credit card statement, tax bill or tax document)

Immunzation information to date
-
Legal Documents (Custody orders, etc)
The following may be required:

Physical Exam Form (Grades: K, 6, 11)

Dental (Grades K, 3, 7)
The child does not need to attend the registration meeting.
