Kindergarten Registration

The following documents are required for registration:

• Child's Birth Certificate

• Parent/Guardian photo ID

• Proof of Residency (current utility bill, lease, mortgage statement, bank statement, credit card statement, tax bill or tax document)

• Immunzation information to date

• Legal Documents (Custody orders, etc)

The following are needed before starting Kindergarten:

• Physical Exam Form

• Dental Exam Form

   

The child does not need to attend the registration meeting.
